Current Emergency Information

At this time, there are no public health emergency events.

Disease Reporting

Disease reporting requires a cooperative relationship between healthcare providers and BCHD so that BCHD can provide the state and the CDC accurate information and keep track of developing issues.

 

There are certain conditions mandated to be reported to us within a specified period of time, with some requiring immediate notification.  The links below will display a list of those conditions, as well as link you to the appropriate form.  The Texas Department of State Health Services (DSHS) has developed forms to be completed.  Those forms are not to be directed to the state, but are to be directed to BCHD so that they can be entered into DSHS' required database.

 

Some of these forms are lengthy and may ask for information you do not have on hand.  Please provide what information you have, and if more information is needed after we receive the form, you will be contacted.

 

When completed, please fax the forms to (979) 864-1501.  This is a confidential and direct fax line to Jan Prejean, RN, the BCHD surveillance nurse.  Per HIPAA law, healthcare providers are not legally required to obtain patient release of records before submitting reportable condition information.
